- All procedures to terminate the activities of all branches, representative offices, and business locations of the cooperative have been completed;
- All debts and other property obligations have been settled;
- There are no ongoing disputes at the Court or Arbitration before submitting the dissolution registration file.

The cooperative commits:
- The information provided in this notification is legal, accurate, truthful, and takes full responsibility before the law for the content of this notification;
- The signatory of this notification is authorized and responsible for performing the registration procedures for the cooperative as prescribed by law and the cooperative's charter.
